The Opp Police Department and OSHA are investigating a Friday morning industrial accident that left a Red Oak man dead.

The OPD responded to a recall at Covington Distribution Services in the 900 block of Douglas Ave. at 10:22 a.m. Friday. Upon arrival, they found a male subject under a forklift.

Fire Department personnel and ambulance crew members began first aid on the victim and he was transported to the Mizell Memorial Hospital by ambulance, where he died of the injuries sustained in the accident.

The victim was identified as Rodney David Maddox, 47, of the Red Oak Community.
